TO REGISTER WITH THE PRACTICE
Geographical Boundary
Each medical practice has a geographical boundary.
Patients living within the boundary are entitled to register with the Kemnay Medical Group.
If you move house and your new home is located outside this boundary, we will ask you to register with another practice.
If you click on the Practice Boundary tab, this can give you access to a map of our geographically boundary and can check if your address falls within our boundary.
If you wish to register with the practice, and you have checked you are within the practice boundary, click on the links below to open and print the registration form.
It would be helpful if you could also print off and complete a medical questionnaire.
This is because it can take several weeks for your past medical records to reach us.
There is now a Patient consent form that you can complete, this allows relatives to access your results/ Medical History and receive/discuss any information with the patient's signed permission, this will also be documented onto the patients file when Clinicians access patients file, so they know that this permission has been given.
Hand the completed forms into reception as soon as possible - don't wait until you are ill before you register!

Consent for Results
All too often patient's ask for information about their family members, which the Reception/Clinician team cannot share, due to confidentiality reasons, this form will prevent any issues or awkwardness to both the patient and staff members.
For GDPR reasons, we cannot give any information including appointment times, test results etc to anyone other than the patient, unless we have written and signed consent from the patient do to so.
This form allows patients who wish specific named family members/representative access to their medical information/results.
If you wish for relative to have access to your information, please download the form and ensure that you circle which information you wish your family members to receive/discuss (Results/Medical History/Medication) then, either post, email or hand this into the reception team and we can then enter this information onto your medical records.

Registration Requirements for New-borns
Any New-born of a currently registered person who is permanently resident within our Practice Boundary, may apply for registration with the practice. It is advised that you register your baby with your GP as early as possible in case you need their help.
Transfer of your GP health records
Most GP health records are a combination of paper records and computer records, created or stored on the practice’s computer system. Your GP practice will hold your records as long as you’re registered with them.
